Vaccine mandates seem to work

You just have to if you want to work here.

That's increasingly the message from hospitals to their vaccine-hesitant staffers, as it becomes increasingly clear that mandates are the only way to make sure the shots become near-universal among them.

The Houston Methodist system went first among U.S. hospitals, announcing a staff vaccine mandate in April. It brooked a lawsuit but also won some impressive numbers: Among 26,000 employees, just 153 resigned or were fired. The rest went from 85% vaccinated to 100%, aside from 600 who were exempted for religious or medical reasons, including pregnancy.

In recent weeks, other big health systems have followed—the University of Pennsylvania, Johns Hopkins, and most recently Michigan-based Trinity Health, with its 117,000 employees across 22 states. Boston-based Mass General Brigham, where 85% of staff had been vaccinated, announced late last month the shots would become mandatory after they win full federal approval.

"We have to protect all of our patients," Mass General Brigham CEO Anne Klibanski said. "We have to protect all of our employees. And that has to be the singular focus of this decision. Is 85% good? Absolutely. Is it good enough? No."

The push for vaccine mandates is spreading beyond the U.S. In France, President Emmanuel Macron this week said Covid shots will be required for healthcare personnel and those working with the vulnerable.

In the U.S., other major hospitals are widely expected to follow suit soon, leaning on widespread experience with mandatory staff flu vaccines. And the vaccine mandate trend is spreading to senior housing and beyond. At Benchmark Senior Living, the biggest assisted living network in New England, the 6,000 employees are on notice that they must be vaccinated—or exempted—by the end of this month.

"How could we look people in the eye who were considering moving Mom or Dad into a community and say that we hadn't taken this step?" Benchmark CEO Tom Grape asked.

Senior housing industry-watchers say vaccine mandates could offer a competitive advantage at a time when occupancy rates are still recovering from the pandemic.

With the delta variant on the rise and concern that cold weather will bring more outbreaks, former New York City associate health commissioner Mark Barnes expects vaccine mandates to spread among office and production workers as well. They are already common at universities.

"We're going to see more vaccine mandates by large organizations of all kinds as the months go by," he said.—Carey Goldberg
